HYSA e-Travel Forms

• Log in to your Affinity Soccer Account to process Travel Notifications and Rosters
• Approved Roster is automatically sent to the email on record. Make sure to request player cards from your league/club registrar if necessary. Travel permission is not required for travel within the State of Hawaii. Medical release forms should be obtained prior to any travel.

HYSA INSURANCE COVERAGE

• On-Line Medical Claim Form (updated 10/6/22) Use this link for players that sustains an injury to a HYSA sanctioned activity (League Games, Sanctioned Tournaments, Training, Practice, Scrimmage, etc.) Be sure to file a claim within 30 days of injury.
